? Nursing: Design Nursing: Design Introduction The s intended to be included in this curriculum are Sexual Assault Nurse Examiner course, Wound Care Course and Foot Care Certificate Courses. These three courses are the need of the hour in today’s world. As there is paramount atrocities against women, providing adequate healthcare and mental support to the victim is the prime objective of every medical practitioner, be it a doctor or nurse. So introducing this course in to the curriculum can train the nurse to be compassionate and treat the patient from a psychological perspective. According to Iwasiw, Goldenberg & Andrusyszyn (2009), curriculum development in nursing education is characterized by ‘interaction, cooperation, change, and possibility conflict, ……..and values of stakeholders’ (Chapter 1, n.p). Wound care management is a hot topic which requires special attention in nurses training. So a program in this would be very useful. Foot care management is providing education and training on proper foot care like foot massaging, trimming toe nails, identifying circulatory disorders etc. All these require expertise. So a course on foot care management is essential. These three areas are in need of experienced man power. So training courses in the above said departments is a necessity. I. SANE Course A. Course Design A course on Sexual assault nursing examination is important as the trauma of a victim is so intense. A four weeks long SANE course should follow a student centered approach with maximum interaction from the part of the student. In addition to the interactive class room sessions the learner should get active clinical practice making the learner competent in sexual assault practice. The learner should be trained in psychological principles to deal with the emotional state of the patient. The class size of 8 students and well trained faculty members makes the course interactive and interesting. Clinical practice sessions and visits to crime lab and crisis centers give the students enough practical exposure. 1. Course Description. This four-week course offers didactic and clinical training for Registered Nurses to conduct a comprehensive sexual assault medical forensic examination and to testify in court on that exam if called. This course should train the nurse in advanced physical assessment skills like emergency care, critical care and maternal child health care. The training they require should meet the requirement of the community they serve as each community varies in the mode of SANE practice. It varies according to the type of patient they handle like adult, adolescent and pediatric. A SANE is trained to work with a multidisciplinary population of professionals like advocates, law enforcement, forensic experts and legal professionals. So training in a collaborative learning environment is a criterion in SANE training. This course provides further knowledge in topics such as forensic code of ethics, extensive forensic examination, and documentation of injuries, collection of forensic samples and its management, testifying legal proceedings, giving emotional support to the victims. 2. Goal. The goal of this course is to mold a registered nurse to a professional trained in clinical preparation and forensic care of a patient who has experienced sexual assault. 3. Objectives. After four weeks of the course; 3.1. The participants will be able to discuss medical forensic history and dynamics of sexual assault 3.2. The participants will be able to describe and give examples of what to document, how to document and management of forensic samples 3.3 The participants will be able to demonstrate or show how to testify in a mock trial and do the GYN exam 3.4. The participants will be able to compare and contrast/ analyze different assault situations. 3.5. The participants will be able to explain/summarize forensic code of ethics 3.6. The participant develops skill in physical examination of the person who has undergone sexual assault 3.7. The participant will be able to assess the risk of pregnancy or contracting a sexually transmitted disease. 3.8. The participant will be able to distinguish the mental trauma of the victim and reduce it. 4. Meetings and Materials. The class will be taught at Montgomery College Science Building Room 402 for didactics and Room 403 for skills lab. At the fourth week, there will be a day field trip to the Crime Lab and Sexual Assault Services in Washington DC. Course book material will be provided. 6. Maximum Class Size: 8 B. Learning Evidence Evaluation The evaluation of the course includes conducting exams testing the learners’ knowledge in examining and managing the sexual assault patient. It should assess the skill of the learner in collecting evidences from the crime scene. Questions regarding the judicial processes associated with sexual assault can also be a part of evaluating the learner. Moral and ethical issues related to sexual assault can be included in evaluating the learner. Formative evaluation takes place through out the course period and summative evaluation occurs at the end of four weeks. ( IAFN. Selecting a SANE Training to Attend). C. Manageable Requirement To ensure whether the students can manage the requirements of the program, the students can be advised to provide their feedback on a regular basis. Based on the feedback constant renewal and up gradation of the course can be done. As SANE course require interaction with the judiciary system the nurses may need contacts outside the hospital premises. These requirements should be meted by the administration as per the requirements of the students. II. Wound Care Course A. Course Design A certified wound care course provides training in monitoring management and treatment of patients with wounds. The course has 4 weeks duration and involves interactive sessions between the learners and teachers. Training in wound care management, wound bed preparation, wound assessment are all part of the course. Other than providing direct care, the course trains the registered nurses to educate patients and care takers in management and prevention of wounds. After a four week long training which includes class room instruction and clinical rotation providing hands on experience the wound care nurse certification is awarded. 1. Course Description. This four-week course is designed to provide RN participants with the best practice in wound care, the science behind healing wounds, and the most current standards of practice. They become specialized in treating and managing wounds and promoting health management practices that prevent their recurrence. Their service is highly valuable as it helps patients to recover from life threatening situations. They are trained in handling bedridden patients suffering from bedsores which are slow in healing. Certified wound care nurses educate people with conditions like diabetes, cancer treatment and people prone to chronic wounds due to limited mobility. They are skilled in debridement, cleaning, bandaging. Along with physician they assess the applicability of surgery, antibiotic therapy and hyperbaric oxygen therapy (Davila, n.d.). 2. Course Goal. The goal of this course is making a registered nurse a professional in wound care management and prevention. Thus making an enormous difference in the lives of patients minimizing their sufferings from critical wounds is intended. 3. Course Objectives. After four weeks of the course, the RN participants; 3.1. will be able to discuss the anatomy & physiology of skin, the physiological process of wound healing, wound management interventions , nutritional management and evidence-based practice guidelines. 3.2. will be able to describe and demonstrate wound management interventions, types of skin ulcers, wound bed preparation, and patient and family education. 3.3. will be able to analyze and explain adjunctive treatment modalities, legal and ethical aspects and quality of care issues, and cost management. 3.4 will be able to communicate positively to patients and care takers regarding management and treatment of wounds. 3.5 will develop skill in wound cleaning and bandaging. 3.6 will be able to apply their knowledge in training nursing assistants who cares bed ridden patients. 3.7 will develop critical thinking, excellent organization and management skills. 4. They join hands with physicians to assess the appropriateness of the antibiotic used. They train nurses and other members of the care team to deal with patients with bedsores and grave wounds. They become specialized in wound care knowledge as well as solid understanding of concepts in general nursing, anatomy, physiology and patient care. Compassion and patience are qualities that develop as part of this training. The evaluation is done formatively and summatively. Summative evaluation includes a certification examination at the end of the fourth week followed by awarding of the certificate. Formative evaluation is done throughout the course by submission of assignments and successful completion of hands on training during clinical rotation. B. Manageable Requirements Constant interaction between the student faculty and the administration is required to ensure the manageability of the program by the students. To know the deficiencies of the program, if any, continuous evaluation measures should be implemented. Above all there should be good rapport between the students and the faculty. III. Foot Care Nurse Course A. Course Design Foot care nurse course is a four week long course providing specialized training in diagnosis management and prevention of problems affecting foot. The class size is 8 and the approach applied is a blend of conservative and contemporary. Classroom instruction is interactive with discussions and practical knowledge is imparted through clinical exposure. The course includes health promotion and client teaching. The course ensures the learning to be a continuous process by constantly updating the knowledge of the learners who take up the course. The course enables the registered nurse to become a foot care specialist in a private setting or with in a health care community setting. 1. Course Description. This four-week course is designed to provide RN participants with the best practice in foot care diagnosing and managing all ailments related foot thereby improving the lives of clients with chronic disabilities. As diabetes is a major risk factor associated with foot problems the course offers comprehensive knowledge in the diagnosis and treatment of type 1 and 2 diabetes. Training in the use of foot care tools and equipments, cutting nails, foot massage, callous and corn removal are imparted during the duration of the course. The students are taught about the different pathological conditions affecting foot, proper nutrition for healthy foot, management And medication for foot related diseases. Training is also given in infection prevention and control. 2. Course Goal. The goal of this course is transforming a registered nurse in to an expert in foot care management. Techniques in foot massaging and reflexology are imparted as part of the course. Even nutritional management for a healthy foot is learned during the course. This awareness from trained foot care nurse can help the aged population in maintaining a healthy foot care regime. 3. Course Objectives. After four weeks of the course, the registered nurse participants will attain the following objectives 3.1. will discuss the anatomy and biomechanics of the foot. 3.2. will explain and identify arterial and venous disease and proper footwear and modification 3.3. will demonstrate how to assess lower extremity neurologic and vascular disease 3.4 will identify and understand the causes of common foot and nail ailments and their prevention. 3.5 will understand the importance of sterilization of equipments used in foot care therapy. 3.6 will recognize the scope of foot care in modern nursing. 3.7 will analyze the factors affecting foot health and the necessity of maintain foot hygiene. 4. Learning Evidence Evaluation The formative evaluation of the learner is done by assessing his performance while handling the foot care tools and equipments and during the management of high risk clients at the diabetic clinic. During class room interaction, knowledge in diagnosis and management of foot ailments are assessed. Summative evaluation is done by an examination at the end of the course and practical testing followed by the awarding of the certificate. The learner expresses his competency in diagnosing managing and treating patients with ease and confidence (Personal support worker). B. Manageable Requirements To ensure whether the requirements of the course are manageable for the students, feedback from the students can be collected through personal interview or by questionnaire distribution. According to the feedback collected modification in the course can be done. Conclusion Sexual Assault Nurse Examiner course, wound care course and foot care course enables a general nurse to establish as a specialist in anyone of these fields. All the three courses are equally important in health care management. These three courses deals with people who require extreme care and attention. The SANE course makes a nurse competent enough to collect forensic evidences and thus collaborates with the legal system other than the hospital set up. This also enhances the clinical skills of the nurse providing opportunities for close physiological examination of the sexually assaulted patient. This course enriches the affective capacities of the individual by developing a compassionate attitude towards the patient. Wound care course equips the nurse with surgical skills. This enables the person to approach wounds with surgical precision. Wound management treatment and prevention is an extensive field which requires the conjoined action of many psychomotor abilities. Apart from these skills the nurse possess the ability to convey the knowledge acquired to care takers and nursing assistants in managing fatally ill or bedridden patients. Foot an often neglected part of the body suffers many problems as one gets aged. So, special attention for this organ is compulsory. A nurse specialized in foot care can handle a series of foot issues ranging from diabetes to plantar fasciitis. Each condition demands a different treatment which only an expert can perform.
